I would say working on this 3rd ball placement won’t be to bad, just working on trying to find the body or crossover point is usually the best way. Of course also trying to use and angles as well to get players out of position but the body is often the best placement. To practice this just keep things simple by serving and trying to focus your placement into the elbow, you can take down the speed a bit if you are finding it hard still.
Watching the opponents bat is a more tricky one and takes practice, multi ball is good for this which you said you you do. Also getting a player to topspin anywhere on the table and you have to keep watching and moving into position to block. This will help you start to watch the bat and read the opponents shot placement.
Last on your recovery and not always being able to play a strong open up, this is something I have had trouble with in the past and I really spent time working on my stepping in and out from the table and using my legs better. What I found is very important is staying low and making sure you get your right foot out after stepping in and then you can keep a good weight transfer in the forehand and still hit with power and not just spin!
